Directly Over the Eyes or Across the Head

You must never place TENS pads directly over your eyes, on your eyelids, or in a position that drives the electrical current straight through your brain (such as placing pads on opposite temples simultaneously).
The sensitive nerves surrounding the ocular cavity and the brain’s internal electrical pathways can be heavily disrupted by external electrical pulses. For safe headache management, pads should be kept exclusively on the back of the neck or along the upper ridge of the shoulders.
The Front and Sides of the Neck (Carotid Sinus)
This is arguably the most critical safety rule of electrotherapy. You must never place TENS pads on the front or sides of your throat.
This specific region houses the carotid arteries and the carotid sinus—sensitive baroreceptors that actively monitor and regulate your body’s blood pressure and heart rate. Passing an electrical current through this area can trick your nervous system, causing sudden, dangerous drops in blood pressure, severe muscle spasms that restrict your airway, or a highly irregular heartbeat.
If you are treating cervical neck pain, always ensure the pads are placed strictly on the soft tissue on either side of the spine at the back of the neck.
Open Wounds, Broken Skin, or Numb Areas
Adhesive TENS pads must only be applied to completely healthy, intact skin. You should never place pads over:
- Open wounds, cuts, or fresh surgical incisions.
- Skin rashes, active eczema, or sunburnt areas.
- Areas affected by deep vein thrombosis (DVT) or active blood clots.
Furthermore, avoid placing pads over areas of skin that are completely numb due to nerve damage or severe neuropathy. If you cannot feel the intensity of the electrical stimulation, you run a high risk of turning the machine up too high and causing localized chemical or thermal skin burns without realizing it.
Directly Over the Spinal Cord or Joints

Electrical currents do not travel effectively through solid bone structures. Placing pads directly over your spinal vertebrae or the hard bony caps of joints (like the kneecap or elbow bone) reduces the effectiveness of the therapy and can cause localized discomfort.
Instead, always position your pads at least 2 to 5 centimeters to the left and right sides of your spine or joints, anchoring them into the surrounding muscle tissue where the nerve pathways actually run.
Directly Over the Heart or Chest Cavity
If you have a pre-existing cardiac condition or wear a pacemaker, you should consult your GP before using a TENS device. For all users, placing electrode pads directly over the chest cavity or directly over the heart muscle is strictly prohibited.
Transthoracic stimulation (passing an electrical current completely through the chest from front to back) can potentially interfere with the natural rhythmic electrical signals of your heart. Keep your pad placements focused on your limbs, lower back, abdomen, and posterior muscle groups.
